One of our clinical liaisons will meet with your loved one’s care coordinator to review their medical history and current health condition. If ContinueCARE is an option, we will work to set an admission date and time that works for your family and care team.
As soon as your loved one arrives, they are assigned a case manager whose goal is to help transition them to the next level of care, based on how much progress they make during their stay.
No. We are a Long Term Acute Care Hospital that specializes in medical management of the complex, critically ill patient. We do offer rehab services as needed and as tolerated for our patients, but are not licensed as an Inpatient Rehabilitation Facility (IRF) that provides more focused and extensive rehab services.
Our patients are treated by physicians with specialized training in LTACH care. All physicians must have privileges with this ContinueCARE Hospital to treat patients here. Following discharge, you will return to your primary care physician for treatment.

Patients receive rehabilitation services 1-3 times per week.

No. We are licensed as a Long Term Acute Care Hospital (LTACH). We provide acute-level, hospital care for a longer period of time than is provided at a short-term acute-care hospital.
Patients come to our hospital because they have an acute illness along with multiple other chronic conditions requiring a prolonged hospital stay. Our specialized services provide the acute care patients need while allowing them more time to heal.
Yes, a physician will see you every day at this hospital. Following discharge, you will return to your primary care physician for treatment.
